Day
Care/Therapy Program Available to Children with Developmental Delays
and Physical and Mental Disabilities.
Mission and Purpose
The mission of Center for Possibilities is to
improve the quality of life for young children and adults with
developmental disabilities and their families. Center for
Possibilities believes that a person’s worth is not diminished by a
disability and that people with disabilities are entitled to full
life experiences. Our goals are to:
- Increase level of independence/highest
level of functioning of our students
- Decrease feelings of loneliness and
isolation for students and their families
- Increase awareness of programs/services
for people with disabilities and their families
- Increase community awareness of the needs
and abilities of people with physical and cognitive disabilities
- Decrease stress on families/caregivers
providing long-term care of children/adults with developmental
disabilities
Center for Possibilities is a not-for-profit
501(c) 3 and is governed by a Board of Directors. Our legal name is
Cerebral Palsy of Northwest Indiana DBA Center for Possibilities.

Programs and Services
Center for Possibilities provides the following
programs and services:

Adult Program for students age 22 and above with a
developmental disability focuses on the development of self-care
skills, daily living skills and academic advancement.
·
Preschool Program 2 for children age 2-3 with a
developmental disability focuses on physical, social and emotional
development while learning how to share, take turns and explore
environment. Toddlers will also be taught basic sign language.
·
Preschool Program 3 for children age 3-4 with
developmental disabilities focuses on learning ABC’s, numbers, and
colors along with fine and gross motor development.
·
Pre-Kindergarten Program for children age 4-6 with
developmental disabilities focuses on kindergarten preparation with
development on pre-writing, pre-reading and counting skills.
·
Physical, Occupational and Speech Therapy to help each
enrolled child (age 2-6) develop skills to function to the best of
their ability in the areas of gross motor, fine motor and
communication skills.
·
Preschool Summer Camps for children with disabilities
who are enrolled in other programs during the school year.
·
Inclusion Preschool Summer Camp is partnership with
the Hobart Family YMCA and the Bubel-Aiken Foundation to increase
inclusion opportunities for children of all abilities.
·
Respite Care provided to families while the
child/adult attends our program.
·
Licensed Social Worker/Family Therapist provides
individual, family and group counseling and information on community
resources.
·
In-house library and resource center containing books
and brochures on resources, services and issues related to
disabilities.
·
Volunteer Program to provide opportunity for the
community to work directly with adults with developmental
disabilities.
·
Community outings to provide experiences for our
students outside of our Center and an opportunity for the community
to interact with and learn about people with developmental
disabilities.
·
In-services for our families and community on services
available to people with developmental disabilities.
·
Family Council and Support Group Meetings for families
to obtain support and encouragement for each other.

Registration
1.
Visit Center for Possibilities and meet with Director or
Admission Coordinator.
2.
Parents/guardians must read and acknowledge by their
signature, that they have read and understand the Parent’s Policy
Agreement thoroughly, reviewing all polices and procedures.
3.
Complete the enrollment application and submit the enrollment
fee of $20.00 for each student. The enrollment fee is a
non-refundable fee and covers one school year. All students must
re-register and complete new enrollment forms at the beginning of
each school year and pay a new enrollment fee. The enrollment fee
is due prior to the student’s first day of school.
4.
Child and Adult Care Food Program (CACFP) forms filled out
and submitted.
5.
New student must submit a copy of a physical that occurred
within the twelve (12) months prior to enrollment.
6.
Children must have a completed and up-to-date immunization
record in our office files at all times. The immunizations may be
obtained through the responsible party’s family physician or at the
local health department. A copy of an immunization waiver for
religious objections (20-8.1-7-2) or exception for child’s health
(20-8.1-7-2.5) is required for students who do not receive
immunizations.
7.
The first weeks tuition for full time students (5 days) is
due before the student’s first day of school. The first months
tuition for part time students (4 days or less) is due before the
student’s first day of school.
8.
All enrollments are done on a first come, first served
basis.
Children will
not be placed on classroom rosters or on waiting list until all
registration paperwork and fees are submitted. This policy also
applies to returning students.

Enrollment
We accept
children (ages 2 thru 6) and adults (age 22 and above) with various
developmental disabilities. There will be no discrimination on
basis of race, color, religion, gender, national origin or
disability.
Our center
shall not admit or maintain any child whose needs it obviously
cannot meet or whose behavior would be dangerous for other children.
(DPW Reg. 470IAC 3-4, 1-7)
Admission
will be based upon:
1.
Children ages 2-5 with a mild, moderate or severe
developmental disability.
2.
Children ages 5-6 with a severe disability.
3.
Adults ages 22 and above with a moderate or severe
developmental disability.
4.
Physical must have occurred within the twelve (12) month
period prior to a new enrollment.
5.
Parent’s signature on all necessary forms
6.
For children- A physical order for
physical/occupational/speech therapy as deemed necessary by child’
physician. A copy of a current immunization record and physical
signed by a physician.

Volunteer
Program
Center for
Possibilities at Cerebral Palsy of NWI, Inc is currently accepting
applications for volunteer positions. These special positions will
be filled with individuals that enjoy being with children and adults
with special needs. No experience required.
Volunteers can have a very flexible schedule varying from 1 hour a
day to 1 day a week or even 1 hour a month. Volunteers will receive
far more than they give including but not limited to smiling faces,
joyful cheers when you arrive, deep appreciation for their time, a
sense of purpose and satisfaction in helping others.

Donations
Center for
Possibilities- Cerebral Palsy of NWI, Inc. is a not-for-profit 501c3
organization that provides services for individuals with
developmental disabilities. It costs $55 per day to provide service
to each child and adult that attends our Center and we only ask for
a $5 a day donation from each family. We receive less than 3% of
our overall budget from State Funding. We rely on support and
donations from the community and business that desire to impact and
improve the lives of individuals with disabilities. Would you like
to share a day, a week or a month with a child or adult with a
developmental disability in need of services that will help them
continue to develop and learn this school year? Your gift will
provide a child with a developmental disability with individual
classroom attention, and necessary Physical, Occupational and Speech
Therapy all in a caring, loving environment. Your gift will provide
an adult with a developmental disability the opportunity to learn
new daily living skills and participate in our community to the best
